This policy explains what information this website collects, how we use it, and the choices you have. It applies to https://cc-philly.com and to messages you send us through it.
Who we are
Care Collaborative provides care navigation, nursing support, and occupational therapy guidance for older adults and their families in Philadelphia & the Main Line. Questions about this policy can be sent to Liz@CC-Philly.com.
What we collect
We collect only what you choose to give us and what is needed to keep the site running:
- Contact form. Your name, email address, phone number if you include it, who the inquiry is about, and your message.
- Email and phone. Anything you share when you contact us directly.
- Technical logs. Our hosting provider records standard server logs, such as IP address, browser type, pages requested, and timestamps, to deliver the site securely and troubleshoot problems.
This website does not use advertising cookies, tracking pixels, or third-party analytics. We do not collect payment information through the site.
Please don’t send medical records through the site
The contact form is for a short introduction only. It is not a secure channel for medical records, insurance details, or detailed health information. We will gather what we need in a private conversation after you reach out. If you include health details in a message anyway, we treat them as confidential and use them only to respond to you.
How we use your information
- To respond to your inquiry and schedule a first conversation.
- To provide and coordinate the services you ask us for.
- To keep the website secure and working properly.
- To meet legal and professional obligations.
We do not sell your information, and we do not use it for marketing lists or send newsletters unless you ask us to.
Who we share it with
We share information only with service providers who help us run the site and respond to you, and only as needed for that purpose:
- Website hosting and form handling (Netlify), which stores form submissions and delivers them to us by email.
- Email providers we use to read and reply to messages.
We may also disclose information when required by law, or to protect the rights, safety, or property of our clients, our team, or others. Once you become a client, how we handle your care information is covered by our client agreement and applicable health-privacy laws.
How long we keep it
We keep contact-form submissions only as long as needed to respond and follow up, and we delete them from the form service periodically. If you become a client, we keep records in line with our professional and legal obligations.
Your choices
- You can contact us by email or phone instead of using the form.
- You can ask us what information we hold about you, ask us to correct it, or ask us to delete it. Email Liz@CC-Philly.com and we will respond within a reasonable time.
Security
The site is served over HTTPS, and form submissions are encrypted in transit and stored by our hosting provider with access limited to our team. No method of transmission or storage is completely secure, which is one more reason we ask you not to send sensitive health details through the site.
Children
Our services are for adults. This website is not directed to children under 13, and we do not knowingly collect information from them.
Changes to this policy
If we change how we handle information, we will update this page and the effective date above.
